I, undersigned individual, parent, and/or guardian, hereby acknowledge and agree:
1. That the activities of spinning, walking, and running may be dangerous and hazardous, both in obvious and unforeseen potential hazards connected therewith. All of these activities as well as others are planned and related to the Thoughts Alive event called Genius Bootcamp, dated as advertised (the “Activities”).
2. That the undersigned, on behalf of himself/herself and his/her children, wards, estate, heirs, executors, and assigns in consideration for being allowed to participate in the Activities acknowledges the physical risks to body and property inherent in pursuing these Activities, and hereby willingly, knowingly, and voluntarily accept and assume the aforementioned risks and responsibilities. The undersigned further acknowledges that the Activities are not required, that if he or she chooses to participate in the Activities, the undersigned participates at his or her own risk, and agrees to abide by any rules, regulations, and/or instructions (verbal or written) given to me with respect to the Activities, if any.
3. That the undersigned, on behalf of himself/herself and his/her children, wards, estate, heirs, executors, and assigns, in consideration for being allowed to participate in the Activities agrees to indemnify and hold harmless Thoughts Alive, LLC, an Arizona limited liability company as well as Leslie Householder, and all of her affiliates, relatives, employees, agents and representatives (collectively the "Owner"), from and against any and all liabilities, claims or damages incurred during any of the Activities as well as any potential liabilities, claims or damages incurred while at the property planned to be used during the conducting of the Activities.
4. The undersigned agrees to indemnify and hold harmless Thoughts Alive, LLC, an Arizona limited liability company and Leslie Householder, as well as any and all of her affiliates, relatives, employees, agents, facilitators and representatives (collectively the "Owner"), from and against any and all liabilities, claims or damages (whether resulting from Owner's negligence or otherwise) arising out of the undersigned's participation in the Activities and use of the event property, including but not limited to the Activities as well as the use and/or provision of any equipment for use by the undersigned during their participation in the Activities and/or stay at the event property.
5. It is the undersigned's specific intent to absolve Owner of all liability and responsibility of any kind in connection with the undersigned's participation in the Activities and/or stay at the event property.
